Could we have additional forms on the page as in the screenshot? and if is yes, could be the answer on the form be linked to the email we have on the page?
For example the (Black background Screenshot) they add all the information and once they press "Submit" all the information will go to my professional email.

Fair-Trade Feet: The plan with the new site designer is to allow you to add form blocks to any page. You could add multiple forms to a single page if you wanted to.

Kris (mymember.site) I came to request this myself - "member info-capture forms" as I would love the ability to collect uselful info from new sign-ups i.e. 'how did you find me' etc... (without needing to message them asking after the event) please please implement this. It can be done externally but I've found sites unreliable or expensive (especially to shedule responses i.e. offering link to this site) - much better if it can be all done in the same place anyway.

Sexactly: We do have plans to add an exit survey, so it would make most sense to have single feature to have a join/cancel survey that can be customized. Perhaps this could be part of the polling feature... not sure.
Are there any other forms that you are looking to add?
